Efficacy and safety of a 4-week course of repeated subcutaneous ketamine injections for treatment-resistant depression (KADS study): randomised double-blind active-controlled trial

Background
Prior trials suggest that intravenous racemic ketamine is a highly effective for treatment-resistant depression (TRD), but phase 3 trials of racemic ketamine are needed.
AimsTo assess the acute efficacy and safety of a 4-week course of subcutaneous racemic ketamine in participants with TRD. Trial registration: ACTRN12616001096448 at www.anzctr.org.au.
MethodThis phase 3, double-blind, randomised, active-controlled multicentre trial was conducted at seven mood disorders centres in Australia and New Zealand. Participants received twice-weekly subcutaneous racemic ketamine or midazolam for 4 weeks. Initially, the trial tested fixed-dose ketamine 0.5 mg/kg versus midazolam 0.025 mg/kg (cohort 1). Dosing was revised, after a Data Safety Monitoring Board recommendation, to flexible-dose ketamine 0.5–0.9 mg/kg or midazolam 0.025–0.045 mg/kg, with response-guided dosing increments (cohort 2). The primary outcome was remission (Montgomery-Åsberg Rating Scale for Depression score ≤10) at the end of week 4.
ResultsThe final analysis (those who received at least one treatment) comprised 68 in cohort 1 (fixed-dose), 106 in cohort 2 (flexible-dose). Ketamine was more efficacious than midazolam in cohort 2 (remission rate 19.6% v. 2.0%; OR = 12.1, 95% CI 2.1–69.2, P = 0.005), but not different in cohort 1 (remission rate 6.3% v. 8.8%; OR = 1.3, 95% CI 0.2–8.2, P = 0.76). Ketamine was well tolerated. Acute adverse effects (psychotomimetic, blood pressure increases) resolved within 2 h.
ConclusionsAdequately dosed subcutaneous racemic ketamine was efficacious and safe in treating TRD over a 4-week treatment period. The subcutaneous route is practical and feasible.
Characterizing the inpatient care of young adults experiencing early psychosis

Introduction/objectives
The available literature suggests that treatments and health services for psychosis are considered to be poorly organized and highly variable. Little is known, however, about how inpatient care is provided to individuals experiencing early psychosis. To facilitate quality improvement activities, we characterized the care this patient group receives in an inner city hospital.
MethodsWe performed chart reviews of individuals admitted to psychiatric inpatient units at St. Paul's Hospital, Vancouver, British Columbia between 01/04/2014 and 31/03/2016. Those who were 17–25 years of age and hospitalized for psychotic symptoms at the time of admission were included. Demographic and health service use were summarized using descriptive characteristics.
ResultsWe identified 73 inpatients (mean age = 22; males = 78%; Caucasian = 41%) that met study inclusion criteria, having a combined total of 102 care episodes and an average length of stay of 30.7 days (median = 18; min = 3; max = 268). Half of the care episodes were repeat admissions, with up to 30% of the patients readmitted within 28 days of discharge. Physical and mental status examinations (MSE) were performed in virtually all care episodes, although frequency is low (31.4% had daily physical examinations and 18.6% had MSE every nursing shift). In 49% and 50% of care episodes, patients were given oral antipsychotics and discharged on depot medications. Even when indicated, not all care episodes had follow-up appointments (60%) or referrals to income assistance (35%), community mental health teams (61%), and housing support (38%).
ConclusionsSpecific programs are needed to address current gaps in inpatient care for patients with early psychosis.

Summary
Johann Wolfgang von Goethe, The Sufferings of Young Werther. Trans. and ed. Stanley Corngold. Norton Critical Editions. New York: W. W. Norton, 2012. 238 pp.
Stanley Corngold's volume for Norton Critical Editions joins Walter Arndt's translation of Faust (edited by Cyrus Hamlin) as the second work by Goethe in this important series. Corngold also translated and edited Kafka's Selected Stories for the series.
To accompany his new translation, Corngold has written an insightful introduction, the flavor of which is manifest in this example: “Note the key terms in this ‘argument,’ for they are original with Goethe: the seat of consciousness is, for want of a more precise physiological or philosophical term, the ‘heart.’ The relation of the self to its intentional object is not that of a concept to a thing but a ‘heart’ to a ‘presence’: the world is ‘present-to’ a type of consciousness other than a conceptual consciousness—one closer to inner sensation than brain or mind” (xii). Rich with perceptive analysis, the introduction as a whole is worth reading even for those well acquainted with Goethe. As “background and contexts,” the book presents relevant excerpts from the correspondence between Goethe and Kestner, lampoons by Nicolai and Thackeray, and helpful sections from Goethe's autobiography.
To represent the voluminous criticism of the novel, Corngold has chosen interesting essays by Harry Steinhauer, Roland Barthes, R. Ellis Dye, David E. Wellbery, Hans Rudolf Vaget, Dirk von Petersdorff, and Christiane Frey and David Martyn. The Barthes text especially gives readers otherwise unaware of the novel's influence a good sense of its importance beyond the German eighteenth century. A chronology and a selected bibliography of English-language criticism of the novel, including ten entries from the Goethe Yearbook, bring the book to a close.
Goethe's first novel has attracted a host of translators, including translations into English by Catherine Hutter (Signet, 1962), Harry Steinhauer (Norton, 1970—like Corngold he translates the title as The Sufferings of Young Werther), Michael Hulse (Penguin, 1989), Victor Lange (Princeton University Press, 1994), and David Constantine, whose translation for Oxford World's Classics appeared almost simultaneously with Corngold’s.
The perils of translation are historically demonstrated by R. D. Boylan's early English version (Bohn's Standard Library, 1854), in which he notoriously has Lotte begging Werther “with broken sobs, to leave her,” not noticing that Werther complies with the request—“bat ihn schluchzend fortzufahren”—by taking up the manuscript to read on.

The Goethe Yearbook is a publication of the Goethe Society of North America, encouraging North American Goethe scholarship by publishing original English-language contributions to the understanding of Goethe and other authors of the Goethezeit while also welcoming contributions from scholars around the world. Volume 21 contains eleven articles, including contributions by leading scholars David Wellbery and Katharina Mommsen; innovative work on the reception of Goethe's works around 1900, on women writers, and on Goethe's contemporary Albrecht von Haller; theoretically sophisticated interpretations, including articles on concepts of space in Alexis and Doraand on notions of sacrifice in Faust; and interdisciplinary pieces ranging from a discussion of contemporary psychological and medical theories of ill humor in relation to Goethe's Werther and an economic reading of Goethe's Faust to an analysis of illustrations of Goethe's works. Compressive strength of hollow microlattices: Experimental characterization, modeling, and optimal design

Recent advances in multiscale manufacturing enable fabrication of hollow-truss based lattices with dimensional control spanning seven orders of magnitude in length scale (from ∼50 nm to ∼10 cm), thus enabling the exploitation of nano-scale strengthening mechanisms in a macroscale cellular material. This article develops mechanical models for the compressive strength of hollow microlattices and validates them with a selection of experimental measurements on nickel microlattices over a wide relative density range (0.01–10%). The limitations of beam-theory-based analytical approaches for ultralight designs are emphasized, and suitable numerical (finite elements) models are presented. Subsequently, a novel computational platform is utilized to efficiently scan the entire design space and produce maps for optimally strong designs. The results indicate that a strong compressive response can be obtained by stubby lattice designs at relatively high densities (∼10%) or by selectively thickening the nodes at ultra-low densities.

Melatonin is useful for recording sleep EEGs: a prospective audit of outcome

The study aimed to determine if melatonin could reliably induce sleep in children undergoing sleep EEG without affecting the usefulness of the EEG itself. One hundred and sixty three children (112 males, 51 females; mean age 8 years, range 1 to 16 years) referred for sleep EEG were studied. The children were given 2 to 10mg of melatonin, depending on age, just before EEG recording. Measurements included sleep-onset latency, adverse effects, and acceptability of the EEG. Usefulness and acceptability of melatonin-induced sleep EEG were compared with the standard technique of sleep EEG following sleep deprivation in 30 children (matched for sex and age). Sleep was obtained in 79% of the 163 children who received melatonin after an average of 33 minutes. Yield of epileptiform abnormalities demonstrated in the melatonin sleep EEG was similar to that reported in the literature for sleep-deprived EEGs. There was no significant adverse effect. When compared, a melatonin-induced sleep EEG was as useful as a sleep-deprived EEG. However, the children's behaviour on the day of the melatonin-induced sleep EEG recording was more acceptable to parents.

The dependence of solid phase epitaxial growth in Si on uniaxial compression applied perpendicular to the amorphous-crystal interface is investigated. Long, thin pure Si bars of square cross section are ion-implanted to produce amorphous layers on the end faces. The bars are placed end-to-end and uniaxially loaded at temperature to partially regrow the amorphous layers. The resulting growth rates are measured ex situ by re-heating the samples on a hot stage and using time-resolved reflectivity to deduce interface depths. Preliminary results are that uniaxial compression is more effective than hydrostatic pressure for enhancing the growth rate, in qualitative but not quantitative agreement with previously made predictions.

The city of Philadelphia is tapping a new source of revenue for $18,000,000 in 1940. Events leading to its adoption have their basis in the past ten years of financial distress. These events were not peculiar to Philadelphia. They may be found in one form or another as budgetary problems in many leading municipalities of the nation. Outstanding among the factors causing distress in Philadelphia was a 25 per cent decline in the assessed valuation of real property since the peak year 1931. This represented over a billion dollars of valuation and a loss of $20,000,000 in current real estate tax levy. Since current real estate has been the chief source of the city's annual income, the decline resulted in curtailment of essential city services and reduction of personnel.
For some years, curtailment of services, reduction in personnel, and the fortunate receipt of large annual sums from delinquent taxes enabled the city to survive the difficulties of the depression. However, the point was reached when these factors no longer sufficed to balance the budget. They were followed by more doubtful financial expedients, namely, overestimating of yearly receipts, floating of current deficits by means of temporary loans, and short-changing of sinking funds through excessive valuation of sinking-fund assets.
